MedReadyMD connects patients with independent licensed healthcare providers through a contracted doctor’s network for telehealth consultations and related healthcare services.

Telehealth services may include:
- Online medical intake forms
- Secure messaging
- Audio consultations
- Video consultations
- Review of medical records, images, and health information
- Prescription evaluation and treatment recommendations, when clinically appropriate

Healthcare providers delivering care through the platform exercise independent medical judgment and are solely responsible for the medical services they provide.

MedReadyMD does not practice medicine or directly provide medical care.

Consent to Telehealth

By using the platform, you voluntarily consent to receive healthcare services through telehealth technologies and electronic communications.

You understand that telehealth may involve the electronic transmission of:
- Medical records
- Personal health information
- Images and photographs
- Prescriptions
- Audio, video, and messaging communications

These communications may be used for diagnosis, treatment, care coordination, follow-up, and prescription fulfillment purposes.

Benefits of Telehealth

You acknowledge and understand that:

- Providers are independently responsible for medical decisions, diagnoses, treatment recommendations, and prescriptions
- MedReadyMD provides administrative, coordination, and technology support services only
- Use of the platform does not guarantee that you will receive treatment or a prescription

Telehealth communications are protected by applicable privacy and confidentiality laws, including HIPAA where applicable.While reasonable safeguards are used to protect your information, no electronic communication system can be guaranteed to be completely secure. By using the platform, you acknowledge and accept the inherent risks associated with electronic communications and data transmission. Patient Acknowledgments

By proceeding with telehealth services through MedReadyMD, you acknowledge that:
- You have had the opportunity to review this consent information carefully
- You understand the nature, benefits, and potential risks of telehealth services
- You understand that telehealth may not be appropriate for all medical conditions or concerns
- You are responsible for providing accurate, complete, and current medical information
- You may withdraw your consent to telehealth services at any time by discontinuing use of the platform or contacting support@medreadymd.com

Withdrawal of consent will not affect any care or services already provided prior to withdrawal.

Availability of Services

MedReadyMD services are intended for patients located in eligible U.S. states where telehealth services may legally be provided. Service availability depends on applicable laws, regulations, and the availability of appropriately licensed healthcare providers within the doctor’s network.Not all products or services may be available in every state or jurisdiction. Availability may change at any time without notice due to legal, regulatory, or operational requirements. Before receiving care or submitting payment, patients will be informed whether services are currently available in their state.
